In consideration of being allowed to participate in any way for the United State Soccer Federation (USSF), the United States Adult Soccer Association, Inc. (USASA), the New Jersey Soccer Association (NJSA), its Affiliated member Leagues, and affiliated Member Clubs/Teams, its related events and activities.
The registering participant, acknowledges, appreciates, and agrees that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. KNOWINGLY AND FREELY ASSUMES ALL SUCH RISKS OF INJURY OR ILLNESS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assumes full responsibility for his/her participation; and,
3. Willingly agrees to comply with the stated and customary terms and conditions for participation. If, however, he/she observes any unusual significant hazard during his/her presence or participation, he/she will bring such to the attention of the nearest official immediately; and
4. For himself/herself and on behalf of his/her heirs, assigns, personal representatives and next of kin, HEREBY RELEASES, INDEMNIFIES, AND HOLDS HARMLESS the USSF, the USASA, the NJSA, the NJSA affiliated member Leagues and member Clubs/Teams, their officers, officials, agents and/or employees, other participants sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as “releasees”, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
THE UNDERSIGNED HAS READ THE ABOVE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTANDS ITS TERMS, UNDERSTANDS THAT HE/SHE HAS GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING/ACCEPTING IT, AND SIGNS/ACCEPTS IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
