In consideration of being allowed to participate in any manner in the athletic/sports program, and related events and activities of the USSF, USASA or the NJSA or its affiliated member Leagues and Clubs/Teams,

The registering Team Officer:

1.

Acknowledges and fully understands that he/she will be personally held responsible of all financial obligations incurred by the Club/Team for which he/she is voluntarily registering, to the New Jersey Soccer Association and any of its Affiliated member League or organization;

2.

Accepts any and all Decisions and Rulings of the USSF, USASA, NJSA and League/Competition of affiliation after exhaustion of any Appeal procedure as outlined in those bodies By-laws, Contracts or Agreements;

3.

Releases, waives, discharges and covenants not to sue the USSF, the USASA, the NJSA, any NJSA affiliated League and its affiliated Clubs/Teams, their respective administrators, directors, agents, coaches, other employees and volunteers of the organizations, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releasees”, from demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the decisions made by the releasee or otherwise;

Will inform all players registered by the Club/Team that they may be individually responsible of their fair share of any unpaid financial obligation incurred by the  Club/Team;

Will go over and will disclose all insurance requirements to each of the players registered by the Club/Team;

THE UNDERSIGNED HAS READ THE ABOVE MEMBERSHIP OBLIGATIONS AGREEMENT, FULLY UNDERSTANDS ITS TERMS, UNDERSTANDS THAT HE/SHE HAS GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING/ACCEPTING IT, AND SIGNS/ACCEPTS IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
